Services for adults with disabilities

The Community Living Fund (CLF) assists individuals with limited incomes to transition out of nursing facilities and back to living independently in the community. CLF also provides services to individuals living in the community who are at imminent risk of institutionalization. CLF staff work to ensure that individuals are connected to all available community resources, stable housing, and appropriate healthcare services to support their ability to live at home safely.

CLF is funded by the City and County of San Francisco. Services are provided by Institute on Aging and three partner organizations: Catholic Charities CYO, Conard House, and HealthRight 360.
